The Health Foundation for Western and Central New York (HFWCNY) is an independent private foundation dedicated to improving the health and health care of the people and communities of Western and Central New York. HFWCNY established this fund to support initiatives that improve health outcomes for vulnerable and underserved populations, including frail elders and children ages birth to five living in communities of poverty.
